Chronic Disease Nurse Clinical Services.
The Chronic Disease Nurse is situated within the Clinical Services team of Winnunga Nimmityjah Aboriginal Health Service, Narrabundah. The Clinical Services team provides a holistic approach to culturally appropriate health care for clients.
The role of the Chronic Disease Nurse is to coordinate and enhance the clinical services offered at Winnunga Nimmityjah Aboriginal Health Service to clients with chronic diseases.
Duties will include:
- Chronic Disease Care Coordination, including initiating care plans and team care arrangements, and coordinating external care including allied health and specialist services
- Provide clinical care including bloods and specimen collection, ECGs, and administration of medication
- Take a leadership role in implementing education and health promotion for clients and the community
- Infection control
- Manage quality improvement activities
- Administrative responsibilities, including the management of recalls, and data entry.
