Referral Templates

The general practice must have signed a memorandum of understanding (MOU) with ACT Medicare Local and use a template that will be provided by GPADS to complete a referral.
GPADS will communicate back to the referring GP within 48 hours of referral via health link.
Referrals will be accepted either electronically (preferred) or by fax. This is a GP to GP referral only.

How to Access a MoU

  • Call ACT Medicare Local on 6287 8050 or 0420 361 490 and ask for two MoU's to be sent to your practice. One MoU will be for the practice to keep and one MoU will need to be sent back to ACTML.
  • This MoU needs to be signed by the practice principles or by the practice manager on their behalf. 
  • We cannot process the GP's referral UNTIL the MoU has been signed.

Electronic Referrals via HealthLink

Using the referral template provided, referrals may be sent electronically via HealthLink Secure Messaging.
Please add the GPADS HealthLink unique identifier or EDI "actgpads" to your clinical software address books or contacts lists.
The GPADS Administration Team will allocate your patient to the available GPADS GP.
The last referral of the day will be accepted at 4pm Monday to Friday. Referrals will be processed by priority of date, time and urgency.

Fax Referrals

If you have no clinical software to support the use of eReferrals you can use the referral template provided and send it via fax to 02 6287 8098. As referring GP, please provide your mobile number (which will remain confidential). This is necessary so: 

  • GPADS GP can call the referring GP if urgent consultation is needed. 
  • GPADS staff can call the referring GP if a GAPDS GP is unavailable to attend within a timely manner.

Acceptance of referral will be confirmed by telephone or fax from GPADS staff to referring GP.
