Population Health Program

What is the Population Health Program?

Population health aims to improve the health of people in the ACT while addressing inequities and the needs of the most disadvantaged. It is used to identify health needs, risk and protective factors, service gaps and the effects of targeted interventions. 

What does the Population Health Program do?

  • Provides information about the prevalence of disease and risk factors in the community 
  • Identifies current utilisation of health services 
  • Assesses the adequacy of present efforts to meet current and future needs 
  • Monitors services or interventions to assess their impact 
  • Identifies service gaps

How can the Population Health Program help you?

  • Providing information about clients of primary health care services 
  • Measuring local health needs, risks and changes over time 
  • Providing evidence of need or service gaps 
  • Measuring the effectiveness of care and new interventions

What has the Population Health Program achieved?

It is a new program that has developed a draft outline of a population health plan for the ACTML, a proposed governance structure to support the planning process and has begun engaging with stakeholders.
